Illustrative operating cases

Two cases. Every handover explained.

01Illustrative scenario

Prescription to documented handover

A clinician prescribes a medication and the patient receives the reviewed quantity from pharmacy.

Prepare first
Medication catalogue, dosage units, valid stock lots, pharmacy roles and charge configuration.
  1. The clinician records the medication request and dosage instructions in the encounter.
  2. The pharmacist reviews the prescription, product and available lot.
  3. Record the actual quantity, days supply, preparation and handover with patient instructions.
  4. Reconcile the dispense record, stock movement and applicable charge; administration is recorded separately if relevant.

Acceptance evidence

The authorized request, dispensed lot, handed-over quantity and stock movement are traceable.

02Illustrative scenario

Partial dispense followed by a return

Only part of a prescribed quantity is available and a later return needs stock and financial review.

Prepare first
Partial-dispense policy, outstanding prescription owner, return and quarantine rules.
  1. Review the prescribed and available quantities with the pharmacist.
  2. Record only the actual dispense and retain responsibility for the remainder.
  3. On return, verify the original dispense and apply the approved inspection and quarantine procedure.
  4. Reconcile accepted stock and financial effects while preserving the dispensing history.

Acceptance evidence

Requested, dispensed, outstanding and returned quantities remain distinguishable.

04

Worked operating scenarios

When the normal path changes.

Partial quantity or unavailable stock

Record the quantity actually dispensed, retain the remaining prescription responsibility and apply approved substitution or replenishment rules.

Expired lot or returned medication

Follow the quarantine and return policy; reconcile the inventory movement and financial effect without deleting the dispense history.

05

Delivery boundary

Know what connects, and what needs approval.

Prescribing, dispensing, administration and stock are linked but separate records. Formulary rules, unit conversions, payer coverage and external dispensers need their own accepted configuration.
